Hospital called our Health Center last week
regarding a patient they were discharging,
on Friday, who has diabetes and no medical
home. In addition, the patient uses 2
different insulins daily and would not be
discharged with the medication.
The hospital was able to set up a Saturday
appointment for the patient at the Pinellas
County Health Department, which would
become his medical home because of the
complexity of his medical issues.
We contacted the hospital, requesting an
early Friday discharge for the patient so that
we could see him for one-time medical
assistance in order to secure the proper
medications for him. Maria Viking, our Nurse
Practitioner, saw him and provided a
sample of insulin, syringes and a
prescription for DAP (Drug Assistance
Program) to start the process for long term
delivery of medications; she also gave him a
prescription for sliding scale insulin which
could be filled at the Health Department
Pharmacy. The patient also saw Sandra
Grosvenor, our Care Coordinator/Health
Educator for diabetic education, information
on classes, and glucose monitoring
supplies.
On Saturday, the Health Department
became his medical home. In the
meantime, three health care agencies
worked in harmony to provide the best care
possible until the patient could be seen at
his primary medical home. Bravo all - that's
how it's supposed to work!
