months, muscle aches, urinary incontinence and increased heart rate. The Health Center did and electrocardiogram
as well as lab work. Also, instructions for discontinuing a herbal tea were given.

The patient returned to clinic for lab review.  He was newly diagnosed with diabetes.  His diabetic test was very high
with an AIC of 12.0 and significant Dyslipedemia (High Cholestrol).  He was started with diabetes & cholesterol
medications, and daily blood sugar checks; the long journey of learning to live with diabetes began.  Our motto is:
Control your diabetes do not let the diabetes control you.  

The Health Center staff reviewed the importance of good blood sugar control, good foot care and how to use his new
glucometer to measure his blood sugar daily.  Eager and appreciative of our care, he listened carefully to all the
information given to him by our medical provider, volunteer nurse and diabetes educator.

The patient kept all his appointments and followed all the directions.  He is a success case; now 3 months later he
is within target - his diabetes test AIC went down to 6.4.  He is feeling great and we are well underway preventing
terrible complications of diabetes such as kidney failure, toe or foot amputations and blindness.
